Complete and sign the application form (for download, please click the button)

Add a letter of recommendation from each of the two sponsors, who must be current members of the IAES (please find IAES members by clicking the "Memberlist" button)

Add an updated curriculum vitae and bibliography

Add the list of endocrine surgical procedures performed in the past six months
Send all documents to iaes.stan@nebsc.com.au.
Please note: in the application form the candidate must confirm that the two sponsors have forwarded their letters of recommendation directly to the office of the Secretary-Treasurer iaes.stan@nebsc.com.au
